About Bridging The Gap Care Management Services

Bridging The Gap Care Management Services provides comprehensive care management to Medicaid recipients in Jefferson. Lewis and St. Lawrence Counties. BTG provides care management services to both children and adults who have two or more chronic medical conditions and/or severe mental health issues.

Through BTG’s relationships with community-based medical, behavioral and social service organizations, we are able to provide comprehensive care management services designed to help our clients to navigate the often complex medical, behavioral and social service systems.

BTG works collaboratively with participating providers and family supports to provide a client-driven plan of care so that all of a client’s needs are addressed in a comprehensive manner.

What we want to accomplish with our clients:

• To connect all providers involved in your care through the coordination of services to ensure that you receive the best possible outcomes.
• Work with you to meet social service needs, including assistance finding housing, transportation to appointments, legal services, and educational and employment support.
• Assist you with reducing the need for hospital and/or inpatient stays and emergency room visits through one-to-one, personalized intervention and education with an assigned Care Manager.
• Improve your overall health outcomes by offering targeted disease-related care for chronic illnesses, the utilization of best practices, and guidance on preventative care.
• Assist with mental health and/or substance abuse disorders by working closely with your mental health and/or substance abuse treatment providers to coordinate services and improve your health.

Bridging The Gap (BTG) Care Management Services’ mission is to provide quality care management services to individuals who have felt the sting of a life of turmoil through sickness, mental illness, or family tragedy. It is our goal to partner with them so that they receive the right services, at the right time and in the right amount while raising them to a new level of self-sufficiency and determination in life.
